Social Prescribing Link Workers

Within the PCN, we have a team of Social Prescribing Link Workers available, whose role is take a holistic approach to patient’s health and wellbeing.

These unique specialists connect people with relevant community groups and statutory services for practical and emotional support, which can include anything from counselling referrals to support with homelessness. Help can also be provided for those suffering with feelings of isolation, deteriorating mental health, debt issues, and the impact of COVID.

Social prescribing works for lots of different people, including those with one or more long-term conditions. They’re also beneficial to those needing support with their mental health, and those who may have complex social needs that affect their wellbeing.

Adult Mental Health Practitioner

Our Advanced Mental Health Practitioner at Redcar Coastal PCN is Pippa Dunn, who has over 25 years-experience in the NHS. In this time, she’s covered everything from nursing to psychology management, and various other areas of expertise relating to adult mental health.

At Redcar Coastal PCN, Pippa provides additional capacity to the existing practice-based mental health service, and offers a combined consultation, advice, triage, and liaison function. She rectifies inappropriate referrals and duplicate work, which boosts the effectiveness of GP practices and enables patients to get the right treatment at the right time.

Dawn’s patient-focussed role is vast, and covers the following:

  • Shared decision-making about self-management
  • Facilitating access to treatment services
  • Providing a range of psychological interventions over the telephone, allowing her to speak to more people over a shorter period of time
  • Face to face clients can also be accommodated at the Ravenscar Surgery between Monday and Wednesday, and Huntcliff Surgery in Saltburn on Thursday mornings
  • Discussions with patients’ medication and liaising with GP surgeries of medical prescribers.

Patients with mental health conditions such as severe anxiety or depression, eating disorders, personality disorders, adult ADHD, PTSD, dual diagnosis, and poor responses to medications or interventions from IAPT can also be treated at Redcar Coastal PCN, as well as those who are in immediate crisis or are having suicidal ideation.
